Most people have dealt with back pain at some point. A bad morning after yard work. A flare-up after a long drive. That kind of thing usually settles down in a few days and gets filed away. Persistent back pain is different. It is there in the morning before anything has happened to cause it. It builds through a normal workday while sitting at a desk. It interrupts sleep. It changes weekend plans. It is always in the background, no matter what else is going on. People try things. Rest. Stretching. Ibuprofen. A new mattress. Some of it helps temporarily. None of it fixes it. That is usually when physical therapy starts making real sense. Why the Core Keeps Coming Up Core strength gets talked about a lot in back pain conversations. Not all of it is useful. But the underlying idea is correct. The spine does not support itself. It relies on the muscles surrounding it to manage load, control movement, and absorb the forces that daily life puts through it. When those muscles are weak, poorly coordinated, or not activating at the right time, the spine takes on more stress than it was built to handle alone. The core muscles most relevant to back pain are not the superficial ones that show up in fitness content. They are the deep stabilizers. The transverse abdominis, the multifidus, the pelvic floor, and the diaphragm. These muscles work together to create stability before movement happens. For people with chronic back pain, they often do not activate properly. Strengthening the wrong muscles in the wrong way does not fix that. It just adds load to a system that is already not working right. What Physical Therapy Does A physical therapist does not hand someone a list of core exercises and send them home. The first step is figuring out what is actually driving the back pain. Disc involvement, facet joint irritation, muscle imbalance, and movement dysfunction. These need different approaches. An assessment that identifies the source shapes everything that follows. From there, treatment is built around what the spine actually needs. Manual therapy addresses joint mobility and soft tissue restrictions directly. Specific therapeutic exercise targets the deep stabilizers rather than just loading the back generally. Movement retraining corrects the patterns that have been putting the spine under stress. The goal is not just less pain during sessions. It is a back that handles daily load without breaking down every few weeks. What People Get Wrong About Core Work Sit-ups and planks are not a back pain program. In fact, for many people with active back pain, loading the spine with traditional core exercises makes things worse before anyone figures out why. The deep stabilizers are not being targeted. The movement patterns driving the pain are not being corrected. Just more load on a system that is already failing. Physical therapy approaches core work differently. The starting point is activation and timing, not strength. Building from there in a sequence that the spine can actually tolerate and adapt to.
