Air at the intake means the intake valve, and air hissing from the oil-fill cap means it's getting past the rings into the crankcase. We follow that with a borescope down the spark-plug hole, where glazed or scored cylinder walls confirm what the leak-down already told us. The bore doesn't lie once the camera is in it.
