• Monitoring and assisting residents in improving daily living skills, personal hygiene, medication adherence and interpersonal effectiveness
• Implementing individual service plans to reinforce positive behaviors and help residents achieve their goals
• Observing behaviors to prevent residents from causing harm to themselves or others and intervening according to established policies when risks occur
• Providing transportation to appointments or activities and accompanying residents on community outings
• Communicating important updates, progress, and setbacks regarding caseload to supervisors and care team
• Maintaining timely, accurate documentation
