A vertigo attack can be genuinely frightening the first time. One second you’re fine, the next the room is lurching sideways and you’re grabbing for something solid, not sure if you’re about to fall or be sick. In the middle of that, people don’t want a lecture on the inner ear. They want to know what to do right now, and what to do so it stops happening. So let’s start there, with the practical stuff.

Getting Through the Attack Itself

When the spinning hits, a few things genuinely help in the moment. Stop moving and find something to hold or lean against. Sitting or lying down lowers your risk of falling, which is the real danger during an attack. Fixing your eyes on a single stationary point can take some of the edge off the spinning. And moving your head slowly, rather than whipping it around trying to make it stop, keeps you from making it worse.

Why It Keeps Happening

Once the attack passes, the real question is why it happened and whether it’ll happen again. That depends on what’s behind it, and there are a few usual suspects.

The most common is loose crystals in the inner ear. There are tiny particles that are supposed to stay in one spot, and when they drift into the wrong place, certain head movements set off those short, violent bursts of spinning. Rolling over in bed. Tipping your head back. Looking up at a shelf. If your attacks are triggered by specific head positions, this is often the reason.

Other times it’s the inner ear getting inflamed, often after an illness, which causes longer, more constant vertigo. Or a balance system that’s dulled over the years, leaving you unsteady more than spinning.

Why You Shouldn’t Just Ride It Out Forever

Here’s what a lot of people do. They figure out which movements trigger it, and they start avoiding them. No looking up. Sleeping propped a certain way. Moving carefully all the time. It feels like managing the problem.

The trouble is that avoidance doesn’t fix it, and sometimes it makes things worse. Your brain never gets the chance to adapt, and your world shrinks as you dodge more and more triggers. The attacks keep lurking, waiting for the moment you forget and move the wrong way. That’s a rough way to live when the actual problem is often very treatable.

How Vestibular Therapy Stops the Cycle

This is where real relief comes in. For the crystal type, a therapist can guide your head through a specific sequence that moves those crystals back where they belong, often settling the whole thing in a visit or two. It can feel almost too simple for how much it helps.

For the other types, therapy retrains your balance system so the dizziness fades over a few weeks, through targeted exercises that teach your brain to compensate. Either way, the point is to stop the attacks, not just brace for them.
